West GTA Stroke Network Sharing Forum: Out-patient and Community Focus

This one day event will highlight initiatives currently happening provincially and regionally that align with best practices in the outpatient rehab and community specifically relating to:

-Patient education/transitions of care

-Access and intensity of community rehabilitation (in home and outpatient)

There will be opportunity for Q and A as well as panel discusson with the presenters. Come and find out what is happening regionally as well as provincially in the fields of OP and Community Rehab as well as assisitng patients with transitions into and out of these programs.
